City of Rockford Forms, Licenses & Permits

All forms, licenses, and permits for the City are available below or in person at Rockford City Hall. Staff can assist in license filing, applying for permits, or completing city forms during normal business hours. Permits can be emailed to  Permits@cityofrockford.org  and all other forms can be emailed to cityhall@cityofrockford.org.
